Welcome

Glengormley Practice, known as GP,  is situated in the centre of Glengormley village. The practice currently serves a population of 8,600 patients and new patients may register at the Practice providing they live within a 7-mile radius. We hope this website will provide you with the information required to make the best possible use of the services provided by our Primary Health Care Team. Our doctors, nurses and all our other staff are dedicated to offering a professional service.

How to Register at the Practice

In order to register at the Practice you will need to complete documentation as requested by Business Services Organisation (BSO).  These documents differ depending on your current GP status, ie - moving from another Practice within GB, or moving from outside GB.

You will need to:

  • Download and complete a HSC form HSC_R1
  • Download and complete our Patient Questionnaire GP Patient Health Questionnaire.xls
  • provide photographic ID
  • provide proof of address 
  • provide other documents as stipulated on HSCR1 form

Please be prepared to give us a brief medical history and let us know if you are currently taking medication.  As it will take several weeks for us to receive your complete medical records, it is helpful if you can provide a brief summary from your previous GP. 

We will complete the registration process on the day and you will receive a medical card from BSO in due course.

Registering a Newborn at the Practice

Following the registration of a newborn baby at the local Birth & Registry Office you will be issued with a yellow registration form.  To avoid delay in your baby accessing healthcare services please leave this form into the Practice as soon as possible.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
