Glengormley Practice - Registration information for new patients

Glengormley doctors' surgery is situated in the centre of Glengormley village. The practice currently serves a population of 8,000 patients and new patients may register at the Practice providing they live within a 10-mile radius.

Please telephone the surgery and the Receptionist will inform you of the necessary documentation you must complete prior to registration.

When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Test Results

The Practice has an electronic link to the laboratory at the local Hospital.

To receive test results, please contact the Practice during normal surgery hours. The receptionist will be happy to inform you of the test results reported by the doctor.

For reasons of confidentiality, information will not be given to another person unless prior permission has been given.
