Our doctors, nurses and all our other staff are dedicated to offering a professional service, DURING THE COVID-19 PANDEMIC.
How to Register at the Practice
Contact the surgery by telephone 02890 848420, the Receptionist will ask you a few questions and advice you how to go about registering at the Practice.
You will need to provide us with:
- Photographic ID
- Proof of address
Please be prepared to give us a brief medical history and let us know if you are currently taking medication. As it will take several weeks for us to receive your complete medical records, it is helpful if you can provide a brief summary from your previous GP.
We will complete the registration process on the day and you will receive a medical card from Business Services Organisation in due course.
Registering a Newborn at the Practice
Following the registration of a newborn baby at the local Birth & Registry Office you will be issued with a yellow registration form. Please bring this form to the surgery where we will register your baby for General Medical Services.
